The information you submit to us is considered protected health information (PHI). We will use this information only when necessary to provide, coordinate, or to manage your healthcare. This includes communicating with the doctor that provided you with the referral to our specialists. We will not disclose your PHI to third parties. It is our utmost priority to maintain your privacy, and in circumstances where your PHI must be shared, we will do so only after receiving your explicit consent.
In order to maintain your privacy and the privacy of other patients’, we do not permit audio or video recordings of any kind while visiting our clinic. As we are dedicated to maintaining every patient’s PHI, violation of this policy may lead to our inability to continue providing care to you.

Any harassment, including verbal and/or physical intimidation, violence, abusive behavior, or discriminatory conduct of any kind will not be tolerated. We appreciate that chronic pain can negatively impact every aspect of your life, leading to severe frustration and hopelessness, however we have a zero-tolerance policy for such behaviors directed towards our team. Violation of this policy will lead to immediate discharge from our clinic and/or notification to the appropriate authorities.
